CBD oil for fibromyalgia: What a new clinical study reveals about pain, sleep, and quality of life

The aim was to investigate whether a cannabis oil with a 1:1 ratio of THC and CBD can be safely used and whether initial positive effects on fibromyalgia symptoms can be observed.
Study design
The researchers conducted a high-quality clinical study:
- randomized
- double-blind
- placebo-controlled
A total of 24 adults diagnosed with fibromyalgia were enrolled. Participants received either:
- a cannabis oil with 10 mg THC and 10 mg CBD per milliliter
- or a placebo
After a four-week dosing phase, a twelve-week treatment phase followed. Neither the participants nor the researchers knew who received which preparation during the study. This makes the investigation one of the scientifically most meaningful study designs.

- 20 to 40 percent achieved an improvement of at least 30 percent
- only 10 to 20 percent an improvement of at least 50 percent

CBD and sleep: Significant improvements in the study

The researchers used the internationally recognized Pittsburgh Sleep Quality Index (PSQI). The results showed:
- significant improvement in sleep quality
- statistically relevant changes after 12 weeks
- large effect sizes in favor of the cannabis group
While the scores of the treatment group improved significantly, the placebo participants showed hardly any changes. These results support the assumption that CBD oil may be helpful for sleep disorders caused by fibromyalgia.
Overall fibromyalgia symptoms significantly reduced
Besides pain and sleep, the scientists also examined the overall disease burden. For this, the Fibromyalgia Impact Questionnaire Revised (FIQR) was used. This questionnaire evaluates, among other things:
- physical function
- daily life limitations
- symptoms
- quality of life
Four times more patients benefited
Particularly interesting: 40 percent of participants in the cannabis group achieved a clinically relevant improvement in their fibromyalgia symptoms. In the placebo group, it was only 10 percent. This means: patients using cannabis oil had a fourfold higher chance of experiencing a noticeable improvement in their health condition.
Improved quality of life through cannabis oil
Chronic pain affects almost all areas of life. Therefore, the researchers additionally examined health-related quality of life. The results showed improvements in:
- physical pain
- social participation
- psychological well-being
- physical performance
- vitality
The areas of pain and social activities benefited particularly strongly.
Were there side effects?
One of the most important questions regarding CBD oil for pain concerns safety. The good news: the treatment was overall very well tolerated.
No serious side effects
According to the study report:
- no life-threatening events
- no serious side effects
- mostly mild complaints
Furthermore, 91.7 percent of participants completed the study. Adherence to therapy was even over 90 percent. This indicates a high acceptance of the treatment.
Important note: The study did not investigate pure CBD oil
An important point is often overlooked. The study did not investigate pure CBD oil, but a cannabis oil with:
- 50 percent CBD
- 50 percent THC
Many positive effects could therefore be due to the interaction of both cannabinoids. In science, this is referred to as the so-called "entourage effect." This means: the results cannot automatically be transferred to over-the-counter CBD products. However, they provide valuable indications that cannabinoids have therapeutic potential in fibromyalgia.

The study provides promising indications that cannabinoid-based therapies could help with fibromyalgia. Particularly clear improvements were seen in:

- sleep quality
- quality of life
- disease burden
Less clear effects were observed for:
- fatigue
- exhaustion
- anxiety
- depression
since only 24 people participated, the authors themselves emphasize that larger studies are necessary.

The current study is among the highest-quality investigations of cannabinoids in fibromyalgia. The results show that a 1:1 THC:CBD oil can:
- reduce pain
- improve sleep quality
- increase quality of life
- was generally well tolerated
Particularly remarkable is that 70 percent of treated participants achieved clinically relevant pain relief. Although further research is necessary, these results strengthen the scientific basis for using cannabinoids in chronic pain syndromes.
